Chief Complaint: 主訴

Right ear pressure, fullness off and on since last week

右耳の圧迫・閉塞感　先週から断続的

History: 経過

1 Ms.X sees Dr.C for ear/hearing evaluation, concerned for intermittent right ear fullness, pressure since 3/19.

X氏がC医師を訪ねる。3月19日から断続的に続く、右耳の圧迫・閉塞感について、耳と聴力の診断をもとめる。

2 Ear pain, ear drainage, hearing loss/change, balance problem/dizziness and bothersome tinnitus are denied.

耳痛、耳漏、聴覚の喪失、平衡感覚の問題／目まい、耳鳴りはない。

3 She admits to seasonal allergies.

季節性のアレルギーがある。

4 Family history of ear/hearing problems, chronic middle ear history/ear surgery, noise exposure, head injury/concussion are denied. 

家族の耳と聴覚の病歴 無し、慢性の中耳の病歴／　耳の手術、騒音、頭のけが／脳しんとう 無し。

5 Medications are verified by Dr. C’s assistant.

投薬はC医師の指導下で行われる。

Audiometry: 聴覚検査

Yields right ear only moderate to mild low frequency sensorineural loss. Left ear hearing is WNL (within normal limits 正常範囲). Word recognition for single words in quiet is good both ears at normal conversation levels.

右耳に軽度から中程度の低周波（長波）の感音性の喪失が認められる。左耳の聴覚は正常範囲。静寂下の単語認識は両耳とも良好で通常の会話レベル。

Recommendations

Return to Dr.C for medical management, retesting as needed and after clear with treatment plan.

Dr.Cを再訪。治療計画の後、必要に応じて再テスト。
